Terms and Conditions:

• The student will adhere to the tutoring schedule once it set forth.
• The student will make every effort to login/arrive on time for each scheduled tutoring session. Center is not responsible for makeup time if a student comes late in the class.
• I further understand that all fees must be paid before a student may begin attending classes.
• There is $35 penalty would be added for a late payment or payment decline and no refund of the fees once a student has attended the first class.
• THERE WILL BE NO INDIVIDUAL MAKE UP SESSIONS OR TUITION REFUNDS FOR CLASS ABSENCES DUE TO ANY OTHER PERSONAL REASON.
• If a student cannot attend any class, Student/Parents’ must inform 24 hrs. before a class to SQUARE D Academy, Inc for makeup, otherwise no make will be done, and you will be charged for those hours
• At least 30-day advanced written notice, prior to the start of the last month, is required to withdraw a student from the program. Otherwise, a full month’s tuition will be charged.
• If a student quits before the class starts, a $75 cancellation fees will be applied.
• A Student is responsible for writing and finishing his/her Homework, SQUARE D Academy, Inc’s Tutors are not liable
• If my kid achieves 800 (perfect) points or excellent score in College board SAT test or ACT Test, I authorize SQUARE D Academy, Inc to use any photos or videos taken of my child to be used to promote the interest of SQUARE D Academy.
• I am giving permission to post my kids’ name or Photo on SQUARE D Academy Bulletin board and/or press release.
• To qualify for the Score Improvement Guarantee, a student must attend all of the program sessions, take all assigned practice tests, and complete all of his/her homework in time without fail. Failure to do that voids the guarantee.
•   I understand and accept the terms and policies of the SQUARE D Academy, Inc as stated above.
